A gritty or sandy feeling in your eyes can be uncomfortable and distracting. While dry eye is a common cause, similar discomfort can sometimes occur with corneal conditions that require closer evaluation. At Sacramento Eye Consultants, we evaluate both dry eye and corneal diseases such as Fuchs’ dystrophy to determine what may be affecting your eye comfort and vision.
Dry eye develops when your eyes do not produce enough tears or when the tear film does not properly lubricate the surface of the eye. Without a stable tear layer, the cornea can become irritated.
Common dry eye symptoms include:
• Burning, stinging or a sandy sensation
• Red or irritated eyes
• Fluctuating or temporarily blurred vision
• Sensitivity to light
• Excess watering as the eyes respond to irritation
• Discomfort that may increase with reading, screen use or dry environments
Symptoms can vary throughout the day and may temporarily improve after blinking or using appropriate lubricating eye drops.
Fuchs’ corneal dystrophy affects the endothelial cells lining the inside of the cornea. These cells normally help remove excess fluid and keep the cornea clear. As the number of healthy endothelial cells decreases, fluid can accumulate and cause corneal swelling.
One of the more characteristic early signs of Fuchs’ dystrophy is blurry or hazy vision in the morning that improves as the day progresses. Patients may also notice increased glare or difficulty seeing clearly around bright lights.
As Fuchs’ dystrophy advances, corneal swelling may become more persistent. In some cases, swelling can eventually cause small blisters on the corneal surface, which can lead to pain or significant irritation.
The sensation alone is not enough to tell the difference. Grittiness without significant vision changes often suggests an ocular surface problem such as dry eye, but persistent morning blur, increasing glare or ongoing hazy vision may warrant evaluation for a corneal condition.
Fuchs’ dystrophy can be identified during an eye examination. At Sacramento Eye Consultants, our corneal evaluations may include specialized diagnostic imaging and endothelial cell measurements to assess the health of the inner corneal layer.
If your eyes consistently feel gritty or uncomfortable, especially when symptoms are accompanied by blurred vision or glare, an eye examination can help identify the underlying cause. Early diagnosis is particularly important with progressive corneal conditions because monitoring changes over time can help guide treatment.
For advanced Fuchs’ dystrophy, our corneal specialists can discuss appropriate management and, when necessary, procedures such as DMEK or DSEK that replace the affected endothelial layer of the cornea.
