Fees

Our office will answer questions you may have about our fees for services. Charges are based on the complexity of the services rendered and on the amount of testing required.

Insurance Payments

We are participating physicians with Medicare and accept Medicare assignment. There are many other providers of health insurance as well, each with its own requirements:

• Medicare - Please bring your Medicare card.

• HMO and Senior Plans - Please bring a referral slip from your referring physician.
  
  
• Sutter Insurance - Sacramento Eye Consultants is a Sub-Specialist with Sutter. Sutter requires a referral from a Sutter General Ophthalmologist indicating a disease of the cornea, glaucoma, or condition requiring an anterior segment specialist.
  
  

• Medi-Cal - Please bring your current Medi-Cal card.

• Other Health Plans - Please bring the appropriate identifying membership card.​​​​​​​

As a service to our patients and after your medical deductible has been met, we will file the first and second insurance forms. A monthly statement will be mailed to you after we have heard from your first insurance company and until the bill has been paid in full.
